North Reading Public Schools’ nurses are committed to supporting the health and well-being of every student. Each school has a full-time registered nurse who provides compassionate, comprehensive care, from routine health assessments to the management of illnesses, injuries, and chronic conditions. Working closely with families, educators, and healthcare providers, our nurses help ensure that all students are healthy, safe, and ready to learn.
Medical Documentation Necessary for each School year
- PHYSICAL EXAMINATIONS
- Required for the following students:
- All incoming Kindergarten students
- All 4th-grade students
- All 7th-grade students
- All 10th-grade students
- All students who are new to North Reading Public Schools
- Any student who wishes to play a High School sport MUST have a current physical on file. NO EXCEPTIONS! This is mandated by the Massachusetts Interscholastic Athletic Association (see the MIAA Manual) and also serves as a safety mechanism for your child.
- Required for the following students:
- All 7th graders are required to have a current Tetanus-Diphtheria Booster (TD or Tdap) and their first dose of MenACWY (meningitis vaccine).
- When students turn 16 years old, they must receive their second MenACWY (meningitis vaccine). Please forward to the Nurse's Office as soon as they receive their immunizations, even in the summer months. Please note that your child will be excluded from attending school if they are not compliant with the Massachusetts State-required immunizations for school attendance.
Medical exemptions (statement from a physician stating that a vaccine is medically contraindicated for a student) must be renewed annually at the start of the school year, and religious exemptions (statement from a parent/guardian stating that a vaccine is against sincerely held religious beliefs) should be renewed annually at the start of the school year.
